portal hypertensive gastropathy with bleeding points


A 70-year-old woman presented with massive melena and was admitted to the hospital. IV fluids and blood transfusion did resuscitation. The patient gives a history of chronic liver disease. Abdominal ultrasonography revealed liver cirrhosis and mild splenomegaly. Hemoglobin level was 4 gm/dl, and INR was 2. The scope revealed severe congestive gastropathy, with multiple areas of oozing blood. The exciting point is that there are no esophageal varices or duodenal congestion. However, the esophagus showed multiple middle and upper-third congestion points.
What are the lines of treatment?

Hemodynamic management, red blood cell transfusion, vasoactive agents (octreotide, terlipressin) and then beta blockers. Pre-transplant evaluation if warranted.
Full dose PPI and sandostatin infusion
