An 84-year-old
man presented with recurrent hematemesis and melena was admitted to the
endoscopy unit. The patient gives a history of multiple upper GI endoscopies
for injection of fundal varices and band ligation of esophageal varices. The
patient underwent upper digestive endoscopy. The scope revealed fundal varices,
esophageal varices, and small antral polyp.
NOTES:
1- You should localize your site very carefully
before using the band rubber as when the rubber band is attached to the
endoscope your vision is tubular and you cannot perfectly localize the site of
the lesion.
2- If the rubber band grasp the polyp but the polyp is not in the center of your view, you can deflate the suction and retry
hoping to centralize the polyp at the center of the rubber band.
3- - to make sure
that the band is fitted to the polyp, you can insert more than one rubber band.
