Bleeding during band ligation
of esophageal varices (an esophageal varix spurter on top of a white nipple#)
the white nipple that bleeds( better the red nipple sign)
A 58-year-old man presented with a
massive attack of hematemesis. After stabilization of the general condition,
the patient underwent upper digestive endoscopy, which revealed large
esophageal varices with a characteristic white nipple sign (best seen at 6 o'clock
during the time of 1.06).
The line of treatment was banding
the varices, including the white nipple.
During the endoscopy and just
before banding the white nipple. A venous spurter from the white nipple (minute1:58)
begins to gush blood into the esophageal lumen. Fortunately, banding of the
spurter was done successfully, and band ligation of the other varices was done.
