Dilemma of IBS and abdominal complaints

now any patient comes to internal medicine clinics for any recurrent complaint and before exclusion of all organic lesions .... considered irritable bowel syndrome. i wonder when sure diagnostic tool for IBS is present, what is the percentage of real IBS CASES and what is the diagnosis of the rest of cases
