Pulsating sub-epithelial esophageal mass#


A 27-year-old woman presented with epigastric pain and vomiting was admitted to the endoscopy unit. The patient underwent upper digestive endoscopy. The scope revealed pan gastric erythematous mottling, and histopathological examination of gastric biopsies confirmed h pylori-associated gastritis. There is a small pulsating lesion in the upper esophagus that was missed during the introduction of the scope into the esophagus. The esophageal submucosal mass was seen during the withdrawal of the scope. In your opinion, is it transmitted pulsation or a pulsating mass? It is just peristalsis or transmitted pulsating from the aorta. The patient hadn’t any symptoms related to this pulsating mass, should we proceed with an investigation or not In your opinion, what is the first investigation you will begin with (computed tomography or endoscopic ultrasonography? Should we use an esophageal withdrawal time# with a specific time (3 minutes as a lower limit).
